This page explains how HealthNest Medical's direct-pay membership, individual visit pricing, cancellations, and patient responsibilities work.

HealthNest Medical membership is not health insurance. It provides only the primary care services specifically described in the final membership agreement.
Important Information Before You Enroll
HealthNest Medical is a direct-pay primary care practice.
Membership is not health insurance.
HealthNest Medical does not bill insurance for membership services unless explicitly stated in a separately approved written policy.
Members may be responsible for services that are not included in their membership.
We encourage patients to consider insurance or other financial protection for services outside HealthNest Medical, such as emergency, hospital, specialty, laboratory, imaging, and prescription services.
This public policy page is a summary. The final membership agreement controls when there is a difference.

Membership pricing and benefits are subject to the final membership agreement and may change only according to applicable policies, required notice, and the agreement terms.

What Your Membership May Include
Membership services are limited to the services specifically described in the final HealthNest Medical membership agreement. The following is a general summary and must be confirmed before enrollment.

Services Not Included in Membership

- Any services not specifically listed in the final membership agreement
HealthNest Medical members are responsible for the cost of services that are not included in the membership agreement, including care received from other providers, facilities, laboratories, imaging centers, and pharmacies. Members are responsible for charges associated with services not included in the membership agreement.
Membership Terms, Billing, and Cancellation
Memberships are month-to-month and renew automatically on the selected monthly billing schedule until cancelled. Prepayment is accepted for no more than 12 months.
Membership fees are due according to the selected billing schedule. A membership may be paused, restricted, or ended in accordance with the final membership agreement if payment is not received.
Members may request cancellation in writing by email, through the patient portal if available, or by contacting the practice directly. HealthNest Medical asks for 30 days' notice, and the practice will provide 30 days' notice if it ends a membership, as described in the final membership agreement.
Add legally approved refund, prorated refund, and failed-payment terms before publishing.
Services are subject to clinical appropriateness, provider availability, scheduling, location, and practice policies.
HealthNest Medical may update its policies and pricing with advance notice as required by the final membership agreement and applicable law.
Patient Acknowledgements
HealthNest Medical membership is not health insurance and does not replace comprehensive health insurance coverage.
Membership does not cover emergency services. Call 911 or go to the nearest emergency department for a medical emergency.
Members are responsible for charges associated with services not included in the membership agreement.
HealthNest Medical may update its policies and pricing with advance notice as required by the final membership agreement and applicable law.
